Continuing Care/ After Care
Post-Treatment Support
Center Point recognizes the overwhelming challenges facing clients as they transition from treatment to independent living. To address this need, a range of Continuing Care and Aftercare services provide relapse prevention support, community integration skills training, and peer affiliation services.
Continuing Care: Recovery Maintenance
Continuing Care services are initiated when participants complete residential or intensive outpatient programs. Weekly groups provide an on-going supportive forum that addresses re-entry issues and concerns. Continuing Care fosters the development of a stable peer support network (including AA & NA), recovery management skills, and provides continuing vocational services.
Family Support
Family reunification is a primary focus at Center Point. In addition to serving individuals, therapeutic and supportive services are provided their families and significant others. Social and recreational events, sponsored by the agency’s Alumni Association, help to facilitate family involvement.
Aftercare Services
A weekly Aftercare Group provides an opportunity for program graduates to address on-going recovery and community integration issues. Graduates are contacted at regular intervals by Center Point to record post-treatment progress and significant changes in their lives. Clients who complete treatment at Center Point become employed tax-paying citizens, active and involved parents, engage in volunteerism, and are registered to vote.
