Case Manager / Outreach Specialist
Overview of the Position:
The part-time (16-20 hours a week) Veteran Case Manager/Outreach Specialist is responsible for the initial contact, engagement and recruitment of veterans for federally funded program for homeless, unemployed veterans in the Urban Austin, TX area. The Case Manager/Outreach Specialist will provide information and referral to veterans and their families about the housing and employment services available through the program and will work with veteran participants to improve their job readiness skills and employability.
Primary Duties:
· Provides outreach to veterans, their families, community based organizations and employers to provide information about available services and to promote accessibility for employment and housing for participants.
· Identifies potential participants through outreach efforts at a broad range of locations frequented by at-risk veterans and through outreach to community services organizations.
· Conducts screening and eligibility review for interested applicants. Develops an Individual Employment Plan with each enrollee to identify needs, barriers to employment and housing and needed resources.
· Provides referrals for emergency services.
· Provides veterans with case management services to ensure identified needs are met.
· Conducts job development services, locating and meeting with potential employers.
· Provides job preparation training for veteran participants, assists with resume development and provides coaching for interviews.
· Conducts job placement services and follow-up case management post hire.
· Maintains accurate and timely documentation in compliance with the requirements of the funder and Center Point.
Position Qualifications:
· A minimum of one year of full time experience as a case manager for vulnerable populations and a minimum of 60 hours of college courses in pursuit of a degree in social work, psychology, counseling or human services.
· Effective written and verbal communication skills.
· Intermediate computer skills.
· A valid driver’s license and proof of current auto insurance.

To act in a manner which is natural, congruent, authentic and without premeditation. When my responses are authentic and balance emotions with ideas and principles, I have spontaneity. Spontaneity is not spontaneous or immediate.
Gratitude
To permit sincere feelings of appreciation and genuine acknowledgement of what we are blessed with and of what others do for us. Gratitude is defined by giving without remembering, receiving without forgetting and remaining humble and unpretentious.
Respect
The act of appreciative consideration for the thoughts, feelings and actions of others. Finding something of value in everyone.
Discipline
Mental attitude harnessing persistent effort. Discipline calls for optimism, confidence, faith, courage, imagination, tolerance, patience and refusing to give up. It is a continuing upward spiral of progress and a willingness to accept the challenge of that which is difficult.
Reciprocate
To give or take mutually: To create connections and networks of mutuality where the exchange of tangible goods or intrinsic service is offered and accepted, which create no lenders and no borrowers. In order to reciprocate, one must give more than that which has been received.
Honor
The special quality of grace which distinguishes us and forces us to act with dignity, respect and esteem. Honor is the reputable code of behavior by which we act and by which we are judged by others.
